Sapphire Center for rehab and nursing is a five-star long term skilled nursing facility that has immediate openings for enthusiastic and intergenic CNA's for all shifts. We are looking for individuals who desire a worthwhile career that has purpose and makes a meaningful difference in someone's life. Under supervised professional and skilled guidance, our CNA's give direct nursing care to assigned residents and perform other services necessary for their personal comfort needs. All new CNAs hired in the nursing department will get a sign on bonus of $1200 (paid after conclusion of 90 day probationary period.

  • Certified Nursing Assistant

    Certified Nursing Assistant

    Reports to: RN Supervisor, RN/LPN Charge Nurse


    Position Summary:

    The certified nursing assistant, under the supervision and direction of a licensed nurse, provides direct and indirect care as delegated and as appropriate to the ages of residents served in areas of clinical practice.


    Duties and Responsibilities:

    • Provides or assists in resident bathing (partial, bed, shower), grooming needs, personal care and other activities of the daily living according to the written plan of care in nursing rehabilitation needs.

    • Prepares resident unit/room to receive resident on admission, transfer, or discharge. Changes bed linen. Handles and appropriately cares for residents clothing and other personal property from admission to discharge.

    • provides or assists with range of motion exercises, lifting, turning, positioning, transferring, ambulating and other mobility needs as directed, using proper body mechanics.

    • Applies and utilizes special equipment, including but not limited to pressure relieving devices, splints, Walker, cane, mechanical lifters, according to instructions.

    • Feeds and assists with nutritional intake, includes ability to follow dietary restrictions, including thickened liquids.

    • Assist with donning and doffing residents orthopedic and prosthetic devices.

    • Transports resident via stretcher, wheelchair, geri-chair or bed. Provides other transport duties required for resident care.

    • Utilizes appropriate precautions and provides general care to residents on precaution.

    • Takes residence temperature, pulse, respiration, height, and weight as directed.

    • Assess the health care team in technical treatments or procedures based on the resident written plan of care and the demonstration of level of competency.

    • Displays adequate judgment to notify the charge nurse of any changes in clinical or functional status.

    • Observes patient behaviors and reports observations to the charge nurse. Reports any incident/ accident with or without injuries promptly to the charge nurse or supervisor.

    • Assist in socialization activities and reports the level of patient participation to the charge nurse.

    • Displays behavioral management skills, including positive reinforcements and adherence to behavioral management program structure.

    • Collect specimens, accordance with facility procedure.

    • Provides postmortem care as per established policy and procedure.


    Quality Improvement:

    • Participates in unit based performance improvement activities, including the implementation of plans for correcting identified variances.


    Risk Management: 

    • Demonstrates awareness of a sensitivity to resident rights. Maintains dignity and confidentiality of the residents and serves as a resident advocate.

    • Provides a safe and secure environment for residents, visitors, staff, and self, through the application of safety principles and precautions.

    • Performs efficiently in emergency situations (remains calm, uses good judgment), follows established protocols and informs appropriate personnel promptly.

    • Performs within the scope of practice and utilizes judgment and action to reduce potential adverse consequences for residents, institutions, and self. Reports promptly to charge nurse, supervisor, of any unusual occurrences, accidents, incidents, involving residents. Never attempt to move a resident following falls/ accidents until seen by licensed personnel.


    Accountability/ Goals: 

    • Assumes responsibility for own practice as defined within the principal duties of the job description, facility policies and procedures.

    • Supports the philosophy and mission of the facility. Participates in the development and attainment of unit and department goals.

    • Response to all emergency codes and alarms, safety of the residents is paramount. Alarms should be accounted for at all times, E. G. Bed alarms and chair checks.

    • Complies with facility dress code and safety policy procedures.


    Self-Development: 

    • Participates in unit-based staff conferences, in service programs and educational activities to meet facility standards.

    • Identifies personal learning needs and seeks opportunities for learning. 

    • Validates knowledge base and skill level by seeking guidance from assisted licensed nurses.

    • Participates in staff development activities for unit and provides support and guidance to colleagues.

    • Assumes responsibility of precepting new staff when applicable.


    Working Relationships: 

    • Communicates all pertinent information including changes in residence condition, problems and/or concerns to the nurse caring for the resident, utilizing appropriate lines of authority in a timely and effective manner.

    • Accepts responsibility for own conduct.

    • Demonstrates ability to cooperate with and assist fellow health team members.

    • Remains flexible in cooperation with scheduling and staffing needs for provision of resident care, including temporary and float assignment.


    Documentation:

    • Records accurate, timely documentation of residence vital signs and weights. Complete clothing/valuables list as per policy. Completes accurate documentation of accountability sheet by the end of shift.


    Other Performance Factors:

    • Does not abuse or take advantage of sick time or personal days.

    • Consistently reports to work on time and is ready to work at the start of the shift.

    • Always provides proper notification when late or absent.

    • Demonstrates reliability when on call. Available to work overtime when necessary. No line demonstrates the ability to recognize priorities and organization of workflow. Able to perform duties independently, with minimal need for direct supervision.

    • Demonstrates willingness to take on additional responsibilities when they can be assumed without adversely affecting quality or quantity of work.

    • Maintains the confidentiality of all residents, facility and/or physician related information.

    • Reports on work well-groomed and in compliance with facility dress code. Where's ID badge while on duty.

    • Demonstrates the ability to interact in a positive and helpful manner with residents, visitors, physicians, volunteers and coworkers.

    • Exhibits a commitment to the values expressed in FM mission statement (i.e. spirit of excellence, genuine caring, compassion and respect for all persons regardless of position).

    • Reflects commitment to building a supportive work environment and maintains a positive attitude at the workplace and towards their job.

    • Projects a good image in their dealing with the public and is willing to make an extra effort to help build a quality and caring health care institution. 

    • Accepts constructive criticism from immediate supervisor and can take corrective action.

    • Demonstrates respectful relationship with supervisors, coworkers and inner department staff.

    • Cooperates with other staff members to ensure maximum resident care and completeness of assignments assist peers with job task; offer to help peers as time permits. Work with others as a team, to ensure residents are out of bed, routine care is being provided and positioning is routinely being done. Bring patient care issues to the attention of the nurse.

    • Participates in clinical quality improvement activities as relevant to role.
